Senior Case Manager Job Description

We are seeking a highly skilled and experienced Senior Case Manager to join our team at Professional Case Management. As a Senior Case Manager, you will be responsible for providing direct care and case management to assigned clients, collaborating with regional directors and other personnel to identify and correct issues, and participating in all Agency performance improvement initiatives.

Key Responsibilities:
  • Provide direct care and case management to assigned clients
  • Collaborate with regional directors and other personnel to identify and correct issues
  • Participate in all Agency performance improvement initiatives
  • Review and evaluate clinical documentation for accuracy and completeness
  • Perform and document supervisory visits as indicated to facilitate problem resolution
  • Assist with marketing activities such as visiting with clients or physicians to discuss Agency programs
Requirements:
  • Graduate of a state-approved school of professional registered nursing
  • Current, unrestricted RN license in the state(s) of practice
  • Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field
  • One (1) year of supervisory and/or case management experience preferred
