ACCS Clinician

We are seeking a skilled and compassionate ACCS Clinician to join our team at Open Sky Community Services. As a key member of our multidisciplinary team, you will work closely with individuals served, providing evidence-based interventions and support to help them achieve their goals and live a fulfilling life.
Key Responsibilities- Develop and implement comprehensive assessments and individualized treatment plans using person-centered planning.
- Build relationships with individuals served and develop shared understanding of their goals and the steps necessary to achieve them.
- Collaborate with care coordination entities, emergency service providers, outpatient clinics, and other providers to ensure the needs of individuals served are fully met.
- Conduct individual sessions, facilitate support groups, and treatment planning meetings with individuals served.
- Provide clinical consultation and training to staff on the integrated team.
We are looking for a candidate with a Master's degree in a social services field and current licensure or license eligibility. Previous experience working with adults with mental health challenges is required, as well as a valid driver's license and acceptable driving record.
