About the Role

We are seeking a skilled Clinical Therapist to join our team at Open Sky Community Services. As a key member of our Adult Community Clinical Services (ACCS) team, you will play a vital role in delivering evidence-based interventions to individuals with mental illness or dual diagnosis.

Key Responsibilities
  • Develop and implement Comprehensive Assessments and Individualized Treatment Plans using Person-Centered Planning.
  • Build relationships with individuals served and develop shared understanding of Person's goals and the steps necessary to achieve goals.
  • Collaborate with applicable Care Coordination Entities, Emergency Service Providers, outpatient clinics, and other providers/entities to share information and ensure that the needs of the person are fully met.
  • Conduct individual sessions, facilitate support groups and treatment planning meetings with individuals served.
Qualifications

To be successful in this role, you will need a Master's Degree in Social Services field with current licensure or license eligibility. Previous experience working with adults with mental health challenges, a valid driver's license, and acceptable driving record are also required.
