Job Summary

We are seeking a highly skilled MDS Coordinator to join our team at Diversicare of Oak Ridge. As an MDS Coordinator, you will play a critical role in ensuring the accuracy and compliance of Minimum Data Set (MDS) assessments, which are essential for providing high-quality patient care.

Key Responsibilities
  • RAI Process Coordination: Oversee the Resident Assessment Instrument (RAI) process to ensure accuracy and compliance with state and federal regulations.
  • Interdisciplinary Collaboration: Work closely with the healthcare team to assess patient needs and develop comprehensive care plans.
  • Care Plan Conferences: Conduct regular care plan conferences with patients, residents, and families to ensure their needs are met and care plans are updated accordingly.
  • Education and Training: Provide education and training to staff on the RAI process and MDS assessments to ensure accurate coding and compliance.
  • Quality Measures: Monitor and analyze quality measures to ensure MDS accuracy and compliance with quality standards.
  • Documentation and Reporting: Maintain accurate and timely documentation and reporting of MDS assessments and care plans.
  • Regulatory Compliance: Ensure compliance with Medicare and Medicaid regulatory guidelines.
Requirements
  • Experience: Two years of experience in MDS coordination or a related field preferred.
  • Licensure: Current registered nursing (RN) license in the state of employment.
  • Knowledge: Working knowledge of the MDS 3.0 and relevant regulations.
