Key Responsibilities:
Provide high-quality, patient-centered hospice care in home or facility settings.
Assess patient conditions, manage symptoms, and administer medication as prescribed.
Collaborate with the interdisciplinary hospice care team to develop and implement individualized care plans.
Educate patients and families on the hospice process, symptom management, and caregiving techniques.
Monitor and report any changes in patient condition, ensuring timely adjustments to the care plan.
Provide emotional and psychological support to patients and their families, maintaining an environment of compassion and trust.
Ensure compliance with all applicable legal and regulatory standards for hospice care.
Qualifications:
Current Registered Nurse (RN) license
Minimum of 2 years of nursing experience, with hospice or palliative care experience preferred.
Excellent clinical and critical thinking skills.
Strong communication and interpersonal skills with a compassionate bedside manner.
Ability to work independently and collaboratively within a multidisciplinary team.
Must have reliable transportation for patient visits.
Work Schedule & Compensation:
Flexible work hours
Part-time or per diem schedules available.
Compensation is competitive and based on experience.
