Job Description

As a Hospice RN Case Manager with Interim HealthCare Staffing, you will play a vital role in providing exceptional hospice care to patients and families in Charleston, Berkeley, and Dorchester Counties.

Key Responsibilities:

  • Maintain a specified case management load.
  • Conduct initial and ongoing assessments of patients' physical and psychosocial status.
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG) and healthcare providers.
  • Coordinate care among IDG members and other healthcare providers to ensure continuous assessment and implementation of the IDG plan of care.
  • Direct and supervise services provided by LPN/LVN, Hospice Aide, and Homemaker.

Requirements:

  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
  • One (1) year of hospice care experience.
  • CPR certification.
