Position Overview:
As a Physical Therapist at Nuvance Health, you will deliver comprehensive services aimed at identifying, preventing, and rehabilitating both acute and chronic physical impairments or discomfort.
Key Responsibilities:
Conduct thorough evaluations and analyses of patients, utilizing therapeutic applications, physical modalities, exercise regimens, and patient/family education. You will take charge of the continuous interdisciplinary assessment and collaboratively develop a personalized care plan with the patient and their caregivers.
Maintain effective communication with the physician overseeing the health plan and other healthcare professionals as necessary, ensuring a cohesive approach to the home health care plan.
Work Schedule:
Available shifts include Full-Time, Part-Time, and Per Diem.
- Full-Time positions are eligible for a $30,000 sign-on bonus.
- Mileage reimbursement provided.
- Tuition reimbursement available.
- Monday to Friday day shift schedule.
- No weekend or holiday shifts required.
- 401k matching offered.
Specific Duties Include:
- Performing physical therapy assessments to evaluate functional limitations, mobility status, endurance, and activities of daily living (ADLs).
- Identifying additional home health service needs for patients and facilitating referrals as necessary.
- Administering services as prescribed by the physician, in alignment with the established care plan, while maintaining open communication with all involved healthcare providers.
- Delivering appropriate therapeutic interventions to alleviate pain, enhance function, and restore independence using suitable equipment and modalities.
- Documenting all evaluation data, treatments, and patient responses to therapy on a visit basis, and contributing to discharge planning.
- Participating in case management meetings with team members to synchronize physical therapy efforts with the overall care plan.
- Providing education and counseling to patients, families, and healthcare personnel regarding treatment protocols, ensuring proper documentation of all instructional sessions.
Equipment Provision:
Arranging or supplying necessary equipment to promote client functionality and independence.
Qualifications:
- Minimum of one year of experience in physical therapy.
- Valid Physical Therapy license is required.

Compensation:
Hourly rate based on experience, starting at $65.01 per hour.
