Renown Health is seeking a skilled Transitional Care Nurse to join our team. As a Registered Nurse in our Transitional Care Unit, you will play a critical role in providing high-quality patient care and ensuring a smooth transition for patients from hospital to home or other care settings.
Key Responsibilities- Provide comprehensive nursing services to adolescent, adult, and geriatric patients, including their families/caregivers.
- Assess, plan, implement, and evaluate individualized patient care plans to achieve optimal patient outcomes.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
- Develop and maintain strong relationships with patients, families, and healthcare providers to ensure continuity of care.
- Stay up-to-date with the latest clinical practices and guidelines to provide evidence-based care.
- Current Registered Nurse licensure in the State of Nevada.
- Valid NV or CA driver's license and ability to meet Renown Health's MVR threshold and vehicle insurance requirements.
- Current BLS certification by The American Heart Association standards.
- Ability to possess, or be able to obtain within 90 days, the computers skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
