Nurse Practitioner

We are seeking a skilled and compassionate Registered Nurse to join our team at Sheboygan Health Services. As a Registered Nurse, you will be responsible for providing high-quality patient care and working collaboratively with our interdisciplinary team to achieve optimal patient outcomes.
Job Summary:The Registered Nurse will be responsible for assessing, planning, implementing, and evaluating patient care plans. You will work closely with our patients, families, and healthcare team to ensure that each patient receives individualized care that meets their unique needs and goals.
Responsibilities:- Assess patients' physical and emotional needs, and develop individualized care plans to meet those needs.
- Implement and evaluate patient care plans, making adjustments as needed to ensure optimal patient outcomes.
- Collaborate with the healthcare team to ensure seamless communication and coordination of patient care.
- Provide education and support to patients and families to promote health and wellness.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Current licensure as a Registered Nurse in the state of Wisconsin.
- Current CPR certification.
- Ability to use necessary supplies and equipment, including feeding pumps, oxygen, treatment trays, and medication carts.
- Ability to calculate drug dosages and administer medications safely.
- Ability to read, write, speak, and understand English.
- Basic computer knowledge and ability to learn company applications.
- Experience in a fast-paced environment, preferably in a post-acute care setting.
