POSITION SUMMARY:

The Clinical Nurse Practitioner Supervisor (CNPS) will collaborate with community primary care physicians to oversee and coordinate participant care in conjunction with the Interdisciplinary Team. The CNPS will focus on managing participants' functional, clinical, and emotional needs, promoting their independence and overall health. This role serves as a vital link between participants, community physicians, and healthcare staff to develop and implement care plans, respond to changes in participants' health status, and ensure the delivery of preventive care services.

KEY RESPONSIBILITIES:

  • Oversee operations at a designated PACE site, managing all participants at that location.
  • Supervise Nurse Practitioners and provide guidance to other clinical staff as necessary.
  • Conduct performance evaluations for staff and offer constructive feedback.
  • Act as Medical Director when required or as assigned.
  • Perform annual wellness evaluations for participants, with increased frequency for those at elevated risk.
  • Provide urgent care services and ensure same-day access for participants.
  • Promptly assess and manage acute changes in participants' health conditions.
  • Review participants returning from hospital stays within five days, including medication and treatment plan assessments, and coordinate follow-up care.
  • Conduct biannual reviews of medical histories and maintain precise diagnostic records.
  • Ensure comprehensive and accurate documentation, including appropriate diagnosis coding.
  • Enhance clinical documentation practices and provide staff feedback.
  • Monitor symptoms and conditions to effectively manage care and treatment.
  • Collaborate with primary care physicians to manage high-risk participants and address care gaps.
  • Participate in Interdisciplinary Team meetings and represent community primary care physicians as needed.
  • Communicate with the Interdisciplinary Team and other staff in accordance with established policies and procedures.
  • Educate participants, families, and staff on care-related topics such as disease management and medication adherence.
  • Adhere to HIPAA regulations and safeguard patient information.
  • Conduct home visits as necessary.
  • Supervise the creation and implementation of care plans, ensuring they cater to individual needs.
  • Oversee home care services provided by the Community Health Network, including scheduling and quality assurance.
  • Provide emergency coverage for primary care providers as needed.
  • Participate in on-call rotation duties.
  • Operate within your authorized scope of practice and fulfill required competencies before working independently.
  • Perform additional assigned duties as required.

QUALIFICATIONS:

  • A minimum of two years of clinical nursing experience in home care, care management, or a related field is required.
  • At least one year of experience working with a frail or elderly population is preferred; if less than one year of experience, appropriate training on working with this demographic must be completed upon hiring.
  • Strong oral and written communication skills, along with effective interpersonal abilities, are essential.
  • High-level analytical and research skills are necessary.
  • Ability to manage multiple projects and meet deadlines is required.
  • Basic proficiency in Microsoft Office is expected.

CERTIFICATIONS:

  • Must be licensed to practice as a Nurse Practitioner in New York.
  • DEA Certification is required.
  • AANP or ANCC Board Certification is necessary.

ADDITIONAL REQUIREMENTS:

  • Must be legally authorized to practice in the state where services will be provided.
  • Must be medically cleared for communicable diseases and have all immunizations current prior to engaging in direct participant contact.
  • Must possess a Medicaid Provider ID and a Medicare Provider ID in good standing.
