Role Summary
Reporting to the Clinic Manager, the Case Manager is tasked with overseeing the healthcare and services provided to patients at CIBD throughout their treatment journey. This role emphasizes the effective use of healthcare resources, guiding a multi-disciplinary care team towards achieving optimal clinical results while complying with the relevant standards and regulations.

Key Responsibilities

  • Assessing patient needs and care plans, coordinating with external healthcare facilities as necessary
  • Collaborating with both internal and external stakeholders to ensure seamless care coordination and fulfillment of orders
  • Exhibiting self-motivation, exceptional organizational abilities, and strong communication skills to deliver outstanding patient care
  • Fostering a cohesive care team and addressing staffing requirements as needed
  • Participating in the growth and improvement of the bleeding disorders and thrombophilia program through outreach initiatives

Qualifications

  • Graduation from an accredited Registered Nurse (RN) program
  • Bachelor of Science in Nursing (BSN) is preferred
  • Possession of an active California State Registered Nurse (RN) License
  • A minimum of 2 years of nursing experience in an acute care or outpatient setting
  • Certification in Basic Life Support (BLS)
  • Completion of HIPAA training
  • Willingness to work onsite

Preferred Qualifications

  • Experience in Case Management
  • Knowledge of CCS/GHPP/Medi-Cal regulations
  • Understanding of third-party payer systems and regulations
  • Background in hematology or oncology nursing
  • Bilingual proficiency in English and Spanish
