Key Responsibilities:

* Develop and maintain strong relationships with residents and their families, providing personalized care and support
* Assess and evaluate resident clinical conditions, documenting findings and notifying providers as needed
* Collaborate with the healthcare team to update and modify care plans as necessary
* Supervise and lead care associates, ensuring high-quality care and service delivery
* Administer medications accurately and timely, according to orders
* Participate in meetings related to resident outcomes and care
* Respond to resident, family, or responsible party inquiries in a timely and professional manner
