Job Summary

CenterWell Home Health is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will play a critical role in helping patients regain their independence and improve their overall quality of life.

Key Responsibilities
  • Examine patients' medical history and evaluate their physical abilities to develop personalized treatment plans.
  • Provide services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities.
  • Work closely with patients, families, and healthcare teams to ensure comprehensive care and optimal outcomes.
  • Stay up-to-date with the latest clinical guidelines and best practices in physical therapy.
Requirements
  • Master's Degree in Physical Therapy (or equivalent)
  • Current license to practice physical therapy and maintains licensure requirements
  • 24 hours of continuing education
  • CPR certification
  • 1 year of experience as a physical therapist
  • Excellent verbal and written communication skills
  • Sensitivity to cultural diversity and ability to make accommodations available to provide services to a diverse population
Preferred Qualifications
  • Doctorate degree in Physical Therapy
  • Bilingual
