As a Community Habilitation Specialist, you will play a vital role in providing one-on-one support to individuals with intellectual and developmental disabilities, empowering them to achieve greater independence and integration into their communities.

Key Responsibilities
  • Adhere to policies, procedures, and regulations to ensure a safe and supportive environment for individuals with intellectual and developmental disabilities.
  • Utilize effective communication and time management skills to ensure seamless delivery of services.
  • Collaborate with a highly effective team to provide comprehensive support and services.
  • Complete mandatory trainings and apply skills to enhance the quality of services provided.
  • Meet and demonstrate OPWDD Core Competencies to ensure high-quality services.
  • Ensure health, wellness, and safety by managing crisis situations and providing appropriate supervision.
  • Transport and escort individuals on recreational activities, medical appointments, and home visits.
  • Build positive relationships with individuals, families, and community partners.
  • Maintain a therapeutic environment through various methods, including Personal Outcome Measures, Person-Centered Thinking, and Informed Decision Making.
  • Ensure person-centered plans are implemented and documented.
  • Complete documentation to ensure compliance and billing.
  • Utilize foundational teaching skills, including EMOE, Task Analysis, Essential Skills, Supports, and Modifications.
Requirements
  • A High School Diploma or GED is required; some college is a plus.
  • Work or life experience within the intellectual/developmental disabilities field is a plus.
  • Ability to support individuals with intellectual/developmental disabilities.
  • Basic written and verbal communication skills.
  • Basic computer knowledge.
  • Valid Driver's License and use of personal vehicle (mileage reimbursed).
  • Tuberculosis test mandatory for most positions.

Compensation
