The Behavioral Health Consultant provides brief, evidence-based assessments and interventions to support functional restoration and improve patient outcomes. This role is part of an integrated care team, working closely with medical providers to develop clear treatment plans and provide consultation services.
Key Responsibilities- Apply biopsychosocial models of human behavior and development to inform treatment plans
- Provide first-line interventions for a broad spectrum of physical and mental health problems in a time-limited setting
- Collaborate with medical providers to develop and implement treatment plans
- Provide patient and family education to promote compliance with medical and behavioral treatment plans
- Teach classes and groups related to integrated behavioral health, such as chronic pain management and stress reduction
- Utilize electronic health records and verbal communication to effectively communicate with medical providers
- Provide consultation and training to other team members on mental health, chemical dependency, and behavioral medicine
- Master's degree in social work or related behavioral health field
- Minimum two years of post-master's practice experience in applied psychotherapy or integrated behavioral health in a primary care setting
- Colorado licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT)
- Ability to function effectively in a fast-paced primary care setting and manage multiple competing demands
- Excellent communication and organizational skills
- Bi- or multi-lingual
