About Us: Home Health Connection is a reputable homecare agency accredited by The Joint Commission, seeking a qualified Nurse Manager to join our team.

Job Summary:

  • Provide management and supervision of private duty nurses and aides.
  • Assist with coordination of clinical services to ensure appropriate service delivery under the direction of the Director of Nursing.
  • Supervise care of patients from admission to discharge for assigned case load.
  • Conduct monthly home visits and document supervisory notes.
  • Develop and update Plans of Treatment for assigned patients every 60 days.
  • Attend scheduled meetings to collaborate with other healthcare professionals to plan and implement therapeutic regimes.
  • Perform skill checks for assigned clinical staff to ensure competency.
  • Review clinical documentation to ensure adequacy and quality of care.
  • Provide reinforcement training for caregivers as required.

Qualifications:

  • Graduate of an accredited School of Nursing (BSN or equivalent preferred).
  • Active Registered Nursing license in Maryland/Virginia.
  • Current CPR certification.
  • Pediatric experience: 1 year (minimum), including pediatric direct patient care within the last 3 years preferred.
  • Strong clinical skills to manage the care of technologically dependent patients with trachs/feeding tubes/ventilators.
