We are seeking a highly skilled Psychiatric Mental Health Nurse Practitioner to join our team at JSA. As a key member of our interdisciplinary team, you will provide comprehensive psychiatric care to our participants, working collaboratively with physicians and other healthcare professionals to ensure the best possible outcomes.
Key Responsibilities- Provide psychiatric consultation, evaluation, and treatment to participants in collaboration with physicians and community-based PACE physicians.
- Conduct comprehensive psychiatric assessments and develop individualized care plans in collaboration with the IDT team.
- Collaborate with physicians and IDT in discharge planning from acute or long-term care settings.
- Refer participants to psychiatrists as indicated and oversee their care in nursing homes, making regular visits as needed.
- Communicate participant changes and collaborate on care planning arrangements with the IDT team.
- Master's degree in nursing with a psychiatric/mental health emphasis from an accredited college or university.
- Certified as a Psychiatric Mental Health Nurse Practitioner.
- Active Psychiatric Mental Health Nurse Practitioner State License without restrictions.
- State CDS Certification and DEA License.
- NPI license.
