Under the direct oversight of the Program Manager, the Client Services Coordinator (CSC) plays a crucial role in orchestrating the services and assistance available to individuals with developmental disabilities and their families, in alignment with the Lanterman Developmental Disabilities Services Act. This role does not entail supervisory duties.

This position involves providing support to clients in the designated service area, which requires travel to various locations. Reimbursement for travel expenses incurred during business-related activities is provided on a monthly basis.

SIGN-ON INCENTIVES:

A $250 sign-on incentive is available upon successful completion of 90 days of employment with satisfactory performance. A $500 sign-on incentive is available after 6 months of employment with satisfactory performance. We encourage candidates to mention any referrals from current employees during the application process.

KEY RESPONSIBILITIES:

Manage daily tasks associated with providing case coordination services for clients. Employ Person-Centered Planning methodologies to develop and implement Individual Program Plans (IPP) for clients, produce progress documentation, and conduct annual evaluations. Continuously assess client progress as outlined in the IPP, making necessary adjustments and creating IPP Addendums as required. Advocate for clients and their families by attending relevant meetings and preparing necessary documentation. Identify and utilize community resources effectively on behalf of clients and their families. Conduct individual or family crisis interventions and document actions appropriately. Timely completion of Special Incident Reports and communication with representatives from other agencies as necessary. Coordinate assessments for clients, including medical, psychological, developmental, educational, and vocational evaluations. Facilitate placement and liaison activities as required. Ensure compliance with Medicaid Waiver standards and timely documentation. Arrange for client placements in residential facilities, day programs, or other training programs as clinically indicated. Conduct facility audits as required and address any identified issues. Identify situations in vendor programs that require attention and make appropriate referrals. Keep the Program Manager informed about caseload status, community activities, and any challenging cases. Participate in case conferences and interagency meetings as assigned. Provide coverage for absent clients as directed by the manager. Act as a mentor for new team members as directed. Complete orientation and training sessions for new staff. Visit assigned residential facilities and day programs as required. Engage in at least one Quality Assurance evaluation or audit annually. Complete administrative tasks, maintain consumer files, and document all activities in compliance with established guidelines. Facilitate the procurement of services identified in the IPP. Prepare for compliance reviews and administrative evaluations. Seek to enhance knowledge and skills through mandatory training and continuing education opportunities. Collaborate effectively with team members, sharing expertise and contributing to a positive team environment. Adapt to changes and manage interruptions effectively. Organize travel efficiently to meet client needs. If bilingual, apply language skills as necessary in all job functions. Utilize office equipment responsibly and maintain a tidy workspace. Follow IRC's protocols and ensure the rights and dignity of clients are upheld. Adhere to IRC's personnel policies and procedures. Perform additional duties as assigned.

MINIMUM QUALIFICATIONS:

Bachelor's degree from an accredited institution and one year of relevant experience in case management or a related field, or a Master's degree in a relevant discipline. Bilingual skills are preferred but not mandatory. Ability to establish effective working relationships and collaborate with others. Demonstrated reliability and punctuality. Ability to follow instructions and communicate effectively. Possession of a valid driver's license and a safe driving record.

CASE MANAGEMENT TRAINEE:

We are also accepting applications for a Case Management Trainee position for candidates who meet the educational requirements but lack the necessary experience. This is a year-long training opportunity.
