About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse to provide one-to-one care to children with complex medical needs in their homes. A Day in the Life of a Home Health Care Nurse

You will start each day by reviewing your client's information and planning your visits accordingly. During your visit, you will provide personalized care, including medication administration, trach and g-tube care, and respiratory therapy. You will also document your interactions and any notable events or observations in our electronic health record system.

Responsibilities and Requirements

  • Providing One-to-One Nursing Care: Deliver high-quality nursing care to our clients in their homes, tailored to their unique needs and preferences.
  • Taking Direction from Clinical Team: Collaborate with our clinical team to ensure seamless communication and coordination of care.
  • Following Plan of Care: Adhere to the established plan of care, making adjustments as needed to meet our clients' evolving needs.
  • Valid Delaware LPN License or Multistate License: Possess a valid Delaware LPN license or multistate license, demonstrating your competence in providing nursing care.
  • G-Tube, Trach, Vent Experience: Have experience with g-tube, trach, and vent care, or be willing to receive training and develop these skills.

Salary Estimate: Based on industry standards and comparable positions, we estimate the salary range for this role to be between $65,000 and $85,000 per year, depending on qualifications and experience.
