  • Assess and update patient care plans to meet individual needs and ensure compliance with regulations.
  • Assign and manage caregivers to deliver effective care and oversee the delivery of care.
  • Make informed admission decisions and instruct and guide clinicians to deliver quality care and achieve best practices.
  • Ensure compliance with regulations and documentation requirements and coordinate communication between team members and caregivers.
  • Participate in business development initiatives to grow the business and supervise clinical employees.
  • Conduct performance reviews to drive growth and development and identify and implement performance improvement strategies to enhance patient outcomes and team performance.

Requirements:

  • Graduate of an accredited School of Nursing.
  • Current state license as a Registered Nurse.
  • Two years as a Registered Nurse and one year of management experience in home care or hospice.
