Job Summary

As a skilled Nurse Care Coordinator with SSM Health, you will play a vital role in coordinating and facilitating interdisciplinary plans of care for patients in their homes, Community-Based Residential Facilities (CBRF), assisted living locations, hospitals, or other settings. Your expertise will ensure that patients receive comprehensive and effective care, while also promoting a seamless transition between healthcare providers.

Key Responsibilities
  • Assess and coordinate patient discharge planning needs with the healthcare team.
  • Perform point-of-care testing according to established policies and procedures.
  • Collaborate with physicians to develop and implement patient care plans, providing regular updates on patient progress and recommending revisions as needed.
  • Coordinate and communicate with multidisciplinary team members and outside agencies to achieve optimal patient outcomes.
  • Complete admission paperwork, verify insurance information, and conduct comprehensive medical assessments using the clinical computer program.
  • Provide skilled nursing care within established patient care plans, including vital sign monitoring, wound care, IV management, ostomy care, blood draws, injections, and physical assessments.
  • Document visit notes, treatments, and care, ensuring timely, accurate, and complete documentation in accordance with regulations.
  • Collaborate with patients, families, and healthcare teams to develop comprehensive care plans tailored to individual patient needs.
  • Respond to patient, family, and healthcare team inquiries, addressing needs, issues, or concerns in a timely and professional manner.
Requirements
  • Graduate of an accredited school of nursing or equivalent education for licensure.
  • No prior experience required.
