Licensed Practical Nurse - Skilled Care Requirements:

Professional Experience Requirements:
  • Minimum of one (1) year of experience in a Long Term Care Facility or hospital setting is required.
Essential Duties and Responsibilities:

As a Licensed Practical Nurse, you will play a vital role in providing compassionate and high-quality care to our residents. Your responsibilities will include:

  • Verifying resident identity before administering medications, ensuring accuracy against the Medication Administration Record (MAR), and documenting administration times.
  • Supervising self-administration of medications by presenting them to residents and observing ingestion or application.
  • Obtaining and recording vital signs (temperature, blood pressure, pulse, respiration rates), weight, food and fluid intake/output as directed.
  • Observing residents for responses to medications and reporting any unexpected reactions to designated personnel.
  • Responding promptly to resident call signals within 7 minutes or less to address their needs.
  • Maintaining a safe environment for all residents.
  • Ensuring resident hygiene by adhering to bath schedules and providing baths/sponge baths as needed.
  • Assisting residents with dressing and toileting as required.
  • Monitoring residents for changes in condition and promptly reporting any unusual occurrences to the Resident Services Director or supervisor.
  • Completing thorough incident reports for any unusual events, changes in condition, or accidents.
  • Supporting meal service by setting tables, serving residents, providing refills, removing dishes, and assisting with dining room cleanup.
  • Maintaining cleanliness of common areas as assigned.
  • Completing all required daily paperwork to document assigned duties for each resident.
  • Participating in mandatory compliance, HIPAA, and state/federal regulatory training programs.
  • Performing other duties as assigned that contribute to the well-being and happiness of our residents.
