At Frontier Home Health and Hospice, we're seeking a dedicated and compassionate RN to join our team of hospice care specialists. As a Hospice Nurse, you'll work closely with patients, their families, and other healthcare professionals to provide high-quality, patient-centered care during the final stages of life.

Our Hospice RNs work in a variety of settings, including private homes, residential care facilities, and nursing centers. They must possess excellent communication skills, be able to work independently, and have a strong understanding of hospice care principles.

Responsibilities include:

  • Providing direct patient care, including administering medications, wound care, and symptom management
  • Developing and implementing individualized care plans in collaboration with the interdisciplinary team
  • Conducting regular assessments and evaluations to ensure patient comfort and well-being
  • Collaborating with other healthcare professionals to coordinate care and address patient needs

Qualifications include:

  • Current RN licensure in the state of employment
  • Minimum of one year of clinical experience, preferably in hospice or palliative care
  • Valid state driver's license and reliable transportation
