Advanced Illness Management Coordinator RN

We are seeking a highly skilled Clinical Operations Coordinator RN to join our team at Illumia. This is a critical role that requires strong organizational and communication skills, as well as a deep understanding of healthcare operations.
Key Responsibilities- Coordinate and manage day-to-day business affairs and clinical activities related to the administration of the Advanced Illness Management (AIM) and Palliative Care department.
- Schedule and manage frequency adherence, clinical coordination, medical record maintenance, and marketing functions.
- Field incoming calls from partners and patients, and manage visit frequency compliance, scheduling PRN visits, and post-hospitalization follow-up.
- Order labs, DME, and imaging, and coordinate results with NPs, enter telephone encounters in partner EMR, and enter routine visit med reconciliation in partner EMR.
- Ensure transitions to HSP referrals to CTC for care type, and assist in identifying HSP eligible patients and HH providers that can accept HH referrals from PCC.
- Bachelor's degree in Healthcare Administration, Nursing, or related field.
- Minimum of three years' experience in healthcare delivery or related business experience.
- Experience in Part B billing setting preferred.
- RN License required.
- Experience with computer applications and Electronic Medical Records.
