About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Hospice of Chattanooga team as a Case Manager. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional end-of-life care to our patients and their families.

Responsibilities
  • Assess patient needs and develop personalized care plans in collaboration with our Clinical Director.
  • Coordinate and deliver high-quality care, ensuring the comfort and quality of life for our patients.
  • Provide education and support to patients and families, empowering them to take an active role in their care.
  • Participate in call duties, demonstrating our commitment to being there when it matters most.
Requirements
  • Current unencumbered license as a Registered Nurse.
  • Minimum one year of experience as an RN in a medical surgical/acute care setting, with hospice experience a plus.
  • Computer proficiency in typing and various programs, including EMR.
  • CPR certification if required by state.
