Interim HealthCare of West Michigan is a leading provider of post-acute care services in West Michigan. We are seeking a Registered Nurse to join our Hospice division as a Case Manager. This role offers flexibility and the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Conduct ongoing assessments of patients' physical and psycho-social status
  • Develop and implement care plans in collaboration with the Interdisciplinary Group, patients, and families
  • Provide education and interventions to manage patients' pain and symptoms
  • Ensure equipment, supplies, and services are available
  • Document patient services accurately and timely
Requirements:
  • RN with active, unrestricted license in Michigan
  • At least one year of relevant experience as an RN
  • Preferred: one year of management experience, one year of home health experience, and current CPR certification
