As a hospice care professional, you'll work in home hospice locations across 7 states. You'll be part of a Great Place to Work Certified organization that values empathy, compassion, and excellent patient care.

Responsibilities

  • Under the physician's order, admit patients eligible for hospice services, assess and evaluate patient needs, and identify mutually agreed-upon goals with patients.
  • Communicate patient status and the need for other disciplines to clinical leadership, attending physicians, and hospice physicians, update care plans on an ongoing basis, and revise and resolve patient problems and goals as changes occur.
  • Complete informational visits and obtain patient consents for hospice admission per office procedure, ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and providing Age-Friendly Care.

Qualifications

  • Current unencumbered registered nurse in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
  • Maintain a valid driver's license and good driving record, with the ability to work in a field setting and make sound nursing judgments.
  • Ability to assess patient needs and formulate individualized patient care plans to meet those needs.

Estimated Salary

The estimated salary for this role is $85,000 - $110,000 per year, depending on experience and qualifications.

Location

This role is available in MI, VA, WI, TX, IN, and IL.

This is a full-time position.
