We are looking for a skilled physician who can lead their care team and work collaboratively with other healthcare professionals to improve health outcomes for seniors.
The ideal candidate will have excellent clinical skills, strong leadership abilities, and a passion for delivering patient-centered care.
Key Responsibilities- Provide comprehensive care to a panel of patients, focusing on building trust and promoting health through accountability and coaching.
- Lead their care team, consisting of care promoters, care facilitators, and care coordinators, to ensure seamless patient care.
- Work collaboratively with other healthcare professionals to improve health outcomes for seniors.
- Develop and implement effective treatment plans, taking into account each patient's unique needs and circumstances.
- Maintain accurate and up-to-date patient records, adhering to all relevant regulations and guidelines.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Current State Medical License or ability to obtain one.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC).
- Ability to maintain confidentiality and handle sensitive patient information.
- Excellent communication and interpersonal skills.
- Able to work independently and as part of a team.
