**Job Description:**
The primary focus of this role is delivering high-quality, compassionate nursing care to patients in their own homes.
Key Responsibilities:
- Conduct comprehensive assessments and develop tailored care plans that cater to the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes and provide education on health management and treatment plans.
- Monitor and document patient progress, responding promptly to their needs and concerns.
Requirements:
- Must be a licensed Registered Nurse with experience in home health care.
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
- Able to adapt to new situations and priorities, with a strong commitment to providing exceptional patient care.
Compensation:
$120,000 - $150,000 per annum, depending on experience and qualifications.
