Hospice Care Manager Job Description

We are seeking a compassionate and experienced Hospice Care Manager to join our team at Humanity Hospice LLC.

Job Summary

The Hospice Care Manager is responsible for providing high-quality patient care and coordination of services to meet the needs of patients and their families. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Patient Care:
  1. Conduct thorough assessments of patients' physical and emotional needs to determine the best course of care.
  2. Develop and implement individualized care plans that address the unique needs of each patient.
  3. Collaborate with healthcare professionals to ensure seamless delivery of care services.
  4. Provide education and support to patients and their families on disease management, symptom control, and end-of-life care.
Communication:
  1. Maintain accurate and detailed records of patient interactions, including progress notes and medication lists.
  2. Communicate effectively with patients, families, and healthcare teams to ensure understanding of care plans and progress.
  3. Foster a culture of respect, empathy, and compassion within the hospice team.
Additional Duties:
  1. Participate in interdisciplinary team meetings to discuss patient care and make recommendations for improvement.
  2. Stay up-to-date with relevant industry developments and best practices through ongoing education and training.
  3. Maintain confidentiality and handle sensitive patient information with discretion.
Requirements:
  • Bachelor's degree in Nursing (BSN) or equivalent experience.
  • Current licensure as a Registered Nurse (RN) in the state of Oklahoma.
  • Minimum 1-2 years of experience in hospice care or a related field.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Able to lift, position, and/or transfer patients as needed.
Compensation and Benefits:

We offer a competitive salary range of $60,000 - $80,000 per year, depending on experience. Additionally, we provide comprehensive benefits, including health insurance, retirement planning, and paid time off.
