Job Summary
- The Care Coordinator will work under the supervision of the Operations Supervisor to evaluate referred clients, verify insurance coverage, develop care plans, and coordinate home care services.
- Responsibilities include admissions, scheduling services, and overseeing direct care workers.
- The Care Coordinator will perform scheduling tasks based on office needs and recruit and manage office staff.
Essential Job Requirements
- Education: High School Diploma or equivalent required.
- Experience: Minimum two years' experience in home care preferred.
Required Skills
- Demonstrated ability to work with personnel in developing service delivery plans.
- Self-directed with the ability to work with little supervision.
- Excellent observation skills, flexible, and cooperative.
- Good oral and written communication and organizational skills.
Preferred Skills
- Previous supervisory experience preferred.
Job Responsibilities
The Care Coordinator will be responsible for:
- Evaluating client needs and developing personalized care plans.
- Verifying insurance coverage and ensuring compliance with regulatory requirements.
