- Overseeing all facets of your designated caseload to ensure seamless service delivery.
- Working closely with a **multidisciplinary team** to create and adjust care plans as necessary.
- Providing guidance and oversight to LPNs and support staff in the delivery of patient care.
- Monitoring and assessing patient conditions, making adjustments to treatment plans as required.
- Assisting patients with their daily living activities and ensuring a safe transition during discharge.
- Possession of a valid **New York State RN License**.
- A minimum of one year of **clinical nursing experience** is required.
