Job Summary

We are seeking a compassionate and skilled Master's Level Licensed Social Worker to join our team of healthcare professionals. As a key member of our hospice care team, you will provide coordinated care to patients in their homes, conducting psychosocial assessments, analysis, counseling, and referrals to meet the unique needs of each patient and their family.

Responsibilities
  • Provide comprehensive psychosocial assessments and develop individualized care plans to address the physical, emotional, and social needs of patients and their families.
  • Conduct counseling sessions to support patients and their loved ones in coping with the challenges of a serious illness.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Develop and maintain relationships with community resources to provide patients and families with access to necessary support services.
  • Participate in ongoing education and training to stay current on best practices in hospice care and social work.
Requirements
  • Master's degree in social work from a State accredited program.
  • One (1) year of experience in a healthcare agency, preferably in a hospice or palliative care setting.
  • Working knowledge of community resources and ability to develop partnerships with local organizations.
  • Excellent interpersonal and communication skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Current State Certification as a Licensed Social Worker.
  • Reliable transportation and a valid driver's license.
