Job Summary

Centerstone is a leading nonprofit behavioral health system seeking a Family Support Specialist to join our team. As a Family Support Specialist, you will provide person-centered assistance to clients in the development of life skills and personal goals. You will coordinate client care with a multidisciplinary team, assist clients in connecting to community resources, and provide crisis intervention/prevention to clients and their families.

Key Responsibilities
  • Coordinate client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client's clinical record, including treatment plan, progress notes, reporting, and correspondence with external agents, ensuring compliance standards.
  • Teach and train clients in activities of daily living, in group and individual settings, complying with clinical documentation requirements, and provide services according to organizational guidelines and/or protocols.
  • Connect clients with community support systems as determined to be appropriate, and maintain regular communications with schools, OFC's, and other family service providers that the client and their family participates with or is receiving services from.
  • Provide telephone and on-site crisis intervention/prevention and follow-up to clients and their families.
Requirements
  • Associate's degree required. Two years of experience or two years of college coursework in a related field may be substituted for degree requirement.
  • Two years of professional experience working directly with youth and families in a child welfare, school, childcare, criminal justice, behavioral health, or related setting preferred.
