Mission Neighborhood Health Center is seeking a highly skilled Registered Dietitian to join our team. As a critical care team member, you will be responsible for assessing the nutritional status of patients, tracking and following up with patients with chronic diseases, and maintaining a panel of Adult Medicine patients enrolled in Diabetes Care Management.
Key Responsibilities:- Obtain, record, analyze, and evaluate nutrition and dietary history of patients.
- Develop a patient-centered plan of nutrition, including dietary consultation to patients and their families on the selection and preparation of food.
- Provide ongoing evaluations of patients' nutritional status and follow up at regular intervals.
- Participate in Care Team meetings and run monthly panel management reports to search for chronic care conditions and enroll patients into Diabetic Care Management.
- Track and follow up with patients enrolled in Diabetic Care Management, refer patients to food programs, and serve as a liaison with these programs.
- Provide self-management education to gestational diabetic patients and monitor their diabetes indicators as needed.
- Participate in monthly department meetings and Quality Improvement activities.
- Bilingual in English and Spanish required.
- Registered Dietitian credential.
- Bachelor's Degree in Nutrition/Dietetics or related field.
- 1-2 years of experience in individual nutritional counseling/group nutrition education preferred.
- Knowledge of diabetes care and diabetes self-management.
- Experience or familiarity with community health centers and/or primary care clinics.
- Competitive salary range of $75,400 to $83,000.
