- Provide health education and training to patients and their families, empowering them to manage their conditions and make informed decisions.
- Collaborate with the care team to address patient concerns, changes in condition, and other issues that may impact care delivery.
- Active, unencumbered LPN license in the state of practice.
- Current CPR certification from a recognized organization.
- Recent TB skin test (within the last 12 months).
- At least six months of prior hands-on nursing experience (LPN/LVN) preferred but not required.
- Reliable transportation for patient visits.
