You will also be responsible for facilitating interdisciplinary care team meetings and informal ICT collaboration.

A successful Care Coordinator Specialist will have a Bachelor's or Master's degree in a relevant field, such as social science, psychology, gerontology, public health, or social work. A minimum of 1-2 years of experience in case management, disease management, managed care, or medical/behavioral health settings is required.
