Hospice Buffalo is seeking a skilled and compassionate Nurse Liaison Specialist to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and support to individuals and their families.

Key Responsibilities
  • Patient Assessment and Care Planning: Conduct comprehensive assessments to identify patients' physical, psychosocial, and emotional needs, and develop individualized care plans to address these needs.
  • Hospice Service Presentation: Present hospice services to patients and families, ensuring that they understand the benefits and eligibility criteria for hospice care.
  • Collaboration and Communication: Work closely with healthcare providers, family members, and caregivers to ensure seamless care transitions and effective communication.
  • Documentation and Reporting: Maintain accurate and timely documentation of patient interactions, assessments, and care plans, and provide regular reports to the interdisciplinary team.
  • Education and Training: Provide education and training to patients, families, and healthcare providers on hospice care, pain management, and symptom control.
Requirements
  • Education and Certification: Graduate of an accredited school of nursing with current RN licensure in the State of New York. Certification in Hospice and Palliative Care Nursing is preferred.
  • Experience: 2 years of nursing practice as a professional nurse is preferred.
  • Skills and Abilities: Strong communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in electronic medical records.
