Key Responsibilities- Patient Assessment and Care Planning: Conduct comprehensive assessments to identify patients' physical, psychosocial, and emotional needs, and develop individualized care plans to address these needs.
- Hospice Service Presentation: Present hospice services to patients and families, ensuring that they understand the benefits and eligibility criteria for hospice care.
- Collaboration and Communication: Work closely with healthcare providers, family members, and caregivers to ensure seamless care transitions and effective communication.
- Documentation and Reporting: Maintain accurate and timely documentation of patient interactions, assessments, and care plans, and provide regular reports to the interdisciplinary team.
- Education and Training: Provide education and training to patients, families, and healthcare providers on hospice care, pain management, and symptom control.
- Education and Certification: Graduate of an accredited school of nursing with current RN licensure in the State of New York. Certification in Hospice and Palliative Care Nursing is preferred.
- Experience: 2 years of nursing practice as a professional nurse is preferred.
- Skills and Abilities: Strong communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in electronic medical records.
