Job Responsibilities

Your primary responsibilities will include interviewing patients, assessing priorities, and documenting case activity. You will also consult with members of the multi-disciplinary team and liaison with community resources to support patient treatment. In addition, you will assist patients and families in obtaining community assistance by referring them to proper resources.

You will develop and maintain working relationships with community health, welfare, and social agencies, seeking creative means to assist patients with their needs. Additionally, you will participate actively with other team members regarding opportunities for improvement in standard work. You will work in a constant state of alertness and safe manner, performing other duties as assigned.

Requirements

A Master's degree in Social Work is required, and no experience is necessary. However, you must be licensed as a Clinical Social Worker (LCSW) in the state where you will be working. You must also have a valid driver's license and a clean driving record.
