Job Summary

We are seeking a skilled Clinical Pharmacist to join our team in a closed-door pharmacy environment in Smyrna, GA. The ideal candidate will have a strong background in long-term care and experience in evaluating physician medication orders.

Key Responsibilities

  • Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.
  • Make recommendations for therapy changes to the physician and facility staff as appropriate.
  • Maintain patient/resident database to include current and past medical history, clinical and laboratory data, and concurrent medication.
  • Ensure laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
  • Collaborate with the healthcare team to provide clinical and technical support.
  • Document all communication with physicians, nurses, and other members of the healthcare team in the medical record.
  • Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
  • Obtain and maintain required documentation in accordance with federal and state regulatory agencies and policy.
  • Inform the Pharmacy Manager of medication-related errors.
  • Assist facility staff with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.

Requirements

  • Bachelor's Degree in Pharmacy.
  • Pharm D preferred.
  • 1+ year experience in long-term care.
  • Valid/Active Pharmacist license in Georgia.
  • Knowledge of pharmacy laws and federal and state regulations concerning facilities and residents.
