Job Summary

We are seeking a highly motivated and compassionate Care Coordinator to join our Addiction Response Team. As a key member of our team, you will play a vital role in connecting individuals with Substance Use Disorders to resources that support their long-term health, treatment, and recovery goals.

Key Responsibilities
  • Enroll individuals in services using appropriate assessment tools and complete GPRA surveys at enrollment and regular 6-month follow-ups.
  • Support individuals in engaging with medical and behavioral health treatment plans, scheduling appointments, and accessing medication refills.
  • Provide health needs assessments, identify risk factors, and develop health plans that integrate treatment and recovery services, including Medication-Assisted Treatment.
  • Monitor members according to NCHA Policy and Procedure guidelines.
  • Participate in telephone contacts, office visits, and other means of connecting with members to reach identified goals.
  • Perform health needs assessments and identify risk factors to develop a healthcare plan.
  • Assist members in scheduling appointments, follow-up care, referrals, medication refills, and other services.
  • Attend member appointments with providers and community resources as needed.
  • Participate in health promotion and health education activities for members as identified in their healthcare plan.
  • Work collaboratively with multidisciplinary teams to support achieving goals and reduce duplication of services.
  • Optimize member and family independence through education, community resources, and support.
  • Review cost-effective care and decrease duplication of services for members.
  • Understand, communicate, and facilitate on behalf of patients with regard to complaints, grievances, and appeal processes.
  • Regular case review with Supervisors and Trainers.
  • Maintain strict confidentiality of member records and communications.
  • Complete Certified Addiction Technician Classes and attend supervision as required.
  • Safely transport members to and from MAT appointments in NCHA company vehicles.
Requirements
  • High School Graduate or GED required.
  • Valid Colorado Driver License and Insurance required.
  • Ability to work independently and in a busy office environment.
  • Reliable transportation.
  • Ability to maintain non-reactive and calm when engaging with individuals who may or may not be intoxicated, belligerent, or have a history of behavioral health issues.
  • Ability to adjust priorities quickly as circumstances and needs dictate for member success.
  • Strong decision-making skills.
  • Strong verbal and written communication skills in-person, phone, and email.
  • Ability to work effectively with diverse populations, including People of Color, LGBTQIA+, Disability, Veteran, Low/No income, and/or experiencing homelessness.
  • Ability to maintain confidentiality and handle sensitive information.
Preferred Qualifications
  • Bilingual preferred.
  • Experience working with Medicaid and Medicare.
  • Experience working with professionals in local and regional health.
  • Computer skills, including navigation of multiple Electronic Medical Record systems and data collection tools.
