Key Responsibilities:
- Manage data entry on multiple spreadsheets to track workflows and outcomes
- Manage Emergency Room spreadsheet and workflow
- Participate in Population Health initiatives, including managing outreach programs, mailings, and patient education programs
- Attend trainings, meetings, and presentations associated with the Care Coordination Department and the Vanguard Medical Group organization as required
- Perform administrative duties, including copying, report collation, organizational tasks, and other duties as assigned by Director
Job Essentials:
- Excellent communication skills (written and verbal) and the ability to build trusting relationships with colleagues and patients
- Empathy, compassion, and an understanding of various cultural and socioeconomic backgrounds in the community
- Creative and resourceful problem-solver
- Excellent time-management skills required; ability to independently complete tasks in a busy medical office environment efficiently and effectively
- Strong skills in Word, Excel, and ability to work with electronic health records
Position Requirements:
- Work experience in medical healthcare setting using electronic healthcare record (EHR) required; college degree preferred
Work Environment:
- Full-time position, onsite from 7am-3pm
- Location: Verona site with occasional travel to other Vanguard Medical Group sites or for professional development training
