Job Summary
We are seeking a skilled Registered Nurse (RN) with expertise in case management to join our team. This role will involve working closely with patients to provide personalized care and support. The successful candidate will have excellent communication skills, be able to work independently, and demonstrate a strong commitment to delivering outstanding patient outcomes.
Key Responsibilities
The primary responsibilities of this role include:
- Providing comprehensive case management services to patients, including assessing their needs, developing individualized care plans, and coordinating with healthcare providers to ensure seamless care delivery.
- Collaborating with multidisciplinary teams to develop and implement effective treatment plans that address the physical, emotional, and social needs of patients.
- Conducting thorough assessments of patients' medical conditions, identifying potential risks, and developing strategies to mitigate these risks.
- Communicating effectively with patients, families, and healthcare providers to ensure that all parties are informed and engaged in the care process.
Required Skills and Qualifications
To be successful in this role, you will need to possess the following skills and qualifications:
- Degree in Nursing (RNs) with current licensure and certification in case management.
- Proven experience in case management, preferably in a hospital or healthcare setting.
- Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare providers.
- Strong analytical and problem-solving skills, with the ability to assess complex situations and develop effective solutions.
Salary Information
This position offers a competitive salary of up to $$31,055.38 per week, reflecting the company's commitment to fair compensation and recognizing the value of our employees' hard work and dedication.
