Key Responsibilities:- Conduct ongoing interdisciplinary assessments of patients to identify their needs and develop comprehensive care plans.
- Provide skilled nursing services, including wound care, medication management, and education to patients and their families.
- Collaborate with the interdisciplinary healthcare team to ensure seamless care coordination and communication.
- Monitor and evaluate patient outcomes, making adjustments to care plans as needed.
- Supervise paraprofessional staff and provide guidance and support as needed.
- Graduate of an accredited school of nursing and current RN license in SC or compact license.
- 2 years' experience as a nurse with 1 year of experience in home health.
- Proficient computer skills and knowledge of workplace software.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- $70,000.00 to $80,000.00 DOE
