Join the University of North Carolina at Chapel Hill as a Dementia Caregiving Specialist 1 and contribute to a pioneering clinical trial evaluating an innovative transitional care intervention for older adults with Alzheimer's disease and related dementias (ADRD) discharged from a skilled nursing facility (SNF) and their caregivers.

Job Summary

The Dementia Caregiving Specialist 1 will collaborate with an interprofessional research team to provide telephone-based support for caregivers of SNF patients with ADRD. This role involves reviewing the SNF discharge plan, evaluating post-discharge needs, and providing education and guidance to caregivers to overcome barriers and engage facilitators. Key responsibilities include:

  1. Independent review of the SNF discharge plan and evaluation of post-discharge needs.
  2. Independent review of caregiver progress implementing the SNF discharge plan and caregiver ability to implement the SNF while supporting the unique needs of the patient with ADRD.
  3. Independent caregiver support, including identifying and resolving care needs, adhering to study protocol, educating caregivers to manage symptoms of ADRD, coordinating care to promote access and respond to emerging challenges, and providing support and community resources to manage caregiver strain.
Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in a discipline related to the field assigned and one year of related training or experience; or equivalent combination of training and experience.
  • Dementia-specific training, including a micro-credential as a dementia caregiving specialist.
  • Broad knowledge of dementia and dementia caregiving best practices.
  • Demonstrated ability to consult with dementia caregivers across diverse situations, coordinate care, collaborate with teammates on projects, and identify and solve problems independently.
  • Strong attention to protocols and ability to balance multiple priorities and a complex timeline.
  • Excellent communication and interpersonal skills, with the ability to work and communicate with diverse populations effectively and professionally.
  • Proficiency with Microsoft Word, Outlook, Teams, and Zoom.
Preferred Qualifications

While not required, candidates with diverse clinical experience across the care continuum (hospital, SNF, home health, assisted living, palliative care, and/or hospice) and experience in phone-based consulting with dementia caregivers are preferred. Additionally, candidates with experience teaching/training dementia caregivers are also preferred.
