Job Summary

We are seeking a highly skilled and compassionate Social Worker - Home Health - MSW to join our team at WVUHS Home Care, LLC. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and families in their homes.

Key Responsibilities
  • Conduct comprehensive assessments to develop safe and realistic discharge plans of care.
  • Collaborate with the care team, including physicians, nurses, and other healthcare professionals, to plan for patient discharge needs.
  • Identify and address psychosocial, emotional, and spiritual needs of patients and families.
  • Develop and implement individualized care plans that address patient needs and goals.
  • Provide counseling and support to patients and families to promote optimal health outcomes.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Master's Degree in Social Work from an accredited school of social work.
  • Current licensure to practice Social Work in the state where services will be provided.
  • Valid driver's license and minimum auto insurance.
  • CPR certification within 30 days of hire date.
Preferred Qualifications
  • Two years of Home Health experience.
  • Experience with counseling individuals or groups.
  • Experience with psychosocial, emotional, and spiritual assessment.

  • Exposure to bodily fluids, human tissues, contagious diseases, sharps, and explosive gases.
  • Exposure to toxic gases, fumes, and odors.
  • Exposure to high stress and constant interruptions.

  • Exposure to radiation from x-rays.
  • Exposure to bad road conditions.
  • Exposure to wind, rain, snow, sleet, mud, high and low temperatures, animals and insects, and hazardous roadways.
  • The majority of work is performed in the patient's home and may require extensive travel.
  • Subject to unpredictable situations. May be exposed to adverse or unpleasant home environments.
