About the Role

We are seeking a highly skilled and compassionate Social Services Director to join our team at Sedgewood Center in Falmouth, ME. As a key member of our interdisciplinary team, you will play a vital role in fostering a supportive and inclusive environment for our patients/residents.

Key Responsibilities
  • Develop and implement comprehensive psychosocial assessments for each patient, identifying social, emotional, and psychological needs and strengths.
  • Collaborate with social services staff, interdisciplinary teams, and administration to promote and protect patient rights, dignity, independence, and psychological well-being.
  • Educate patients and families regarding their rights and responsibilities, healthcare decision-making, and available community resources.
  • Provide therapeutic interventions to assist patients in coping with their transition and adjustment to our nursing center.
Qualifications
  • Bachelor's degree in social work or human services required.
  • Must possess any certifications/licensures as required by state of employment to practice in long-term care.
