Social Services Director

We are seeking a highly skilled and compassionate Director of Social Services and Patient Care to join our team at Sedgewood Center in Falmouth, ME. As a key member of our interdisciplinary team, you will play a vital role in creating a supportive and inclusive environment that enables our patients to thrive.
Key Responsibilities:- Develop and implement comprehensive social services programs that promote patient well-being and independence.
- Collaborate with interdisciplinary teams to identify and address patient needs, ensuring seamless care coordination.
- Supervise and mentor social services staff, providing guidance and support to ensure exceptional patient care.
- Maintain accurate and up-to-date records of patient assessments, care plans, and progress.
- Stay current with federal, state, and local regulations, ensuring compliance with all relevant guidelines and standards.
- Bachelor's degree in social work or human services required.
- Must possess any certifications/licensures as required by state of employment to practice in long-term care.
