Job Overview

Responsibilities

* Provide direct patient care, including administering medications, treatments, and therapies
* Develop and implement individualized care plans to meet the unique needs of each patient
* Collaborate with interdisciplinary teams to ensure seamless care coordination
* Educate patients and families on disease management, self-care techniques, and prevention strategies
* Maintain accurate and detailed records of patient interactions and care provided

Requirements

* Active Nursing License in good standing
* Six months of recent experience as a Nurse in a clinical care setting (preferred)
* Home health experience (preferred)
* Strong attention to detail and time management skills
* Effective problem-solving and conflict resolution abilities
* Excellent communication and organizational skills
