About the Role

We are seeking a highly skilled Licensed Professional Counselor to join our team at Rehab Without Walls Neuro Rehabilitation. As a Licensed Professional Counselor, you will play a vital role in providing comprehensive evaluations, assessments, and treatment plans for patients with traumatic or acquired brain injuries or related neurological conditions.

Responsibilities
  • Conduct comprehensive evaluations and assessments to identify patient needs and develop measurable objectives and goals.
  • Develop and implement treatment plans to achieve maximum results in independence and/or function.
  • Provide empirically based psychological interventions to patients and families as appropriate for their specific needs.
  • Design community reintegration activities to assist patients with their psychological/cognitive needs during physical activity.
  • Assist patients in understanding their needs and limitations and how to manage them during function.
  • Regularly re-assess the effectiveness of treatment plans, progress of patients towards goals, and the need for modifying goals or treatment.
  • Communicate patient needs and progress to the treatment team, physician, patient, family members, and others as applicable.
  • Consult with medical providers regarding patient evaluation & treatment, progress of the patient & discharge from treatment.
  • Observe/document and report changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other treatment team members.
  • Maintain and submit evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations.
  • Adhere to standards of the Company's Code of Business Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.
Qualifications
  • Master's or doctoral degree in counseling or a counseling-related field from an accredited college or university, as determined by the state licensing requirements.
  • Current professional license and/or certification as required by the state of practice.
  • A minimum of one year of post master's degree clinical experience and counseling individuals with traumatic or acquired brain injuries or related neurological conditions.
  • Knowledge of complex neurologic injury rehabilitation techniques.
  • Demonstrates effective and professional interpersonal, verbal, and written communication skills.
  • Able to work independently and part of a team.
  • Has reliable transportation and can travel to/from multiple assignments/locations on a daily basis.
